VITA Registration

IF YOU ARE PLANNING TO SCHEDULE APPOINTMENTS FOR MORE THAN ONE TAX RETURN AN INTAKE MUST BE COMPLETED FOR EACH TAX RETURN AND AN APPOINTMENT MUST BE SCHEDULED FOR EACH ELIGIBLE TAX PAYER.

FORM COMPLETION STEPS:

  1. Download the form by clicking the "DOWNLOAD FORM" button below.


  1. When prompted please click SAVE or OPEN

    NOTE: 
    Chrome users should select Show in Folder, and then open the file from the folder location.

  2. Complete the entire form, and click the Save icon.

  3. If prompted to replace existing file, please select Yes.

  4. Return to this VITA Registration page.

  5. Click the "Browse" button below, and navigate to your form save location.  Click or (Double-click) to select your form.

What to Bring to Your Local VITA Site:

The following items are required to receive VITA services:

  • Valid Photo ID for you (and your spouse if married filing jointly)

  • Social Security Cards for you, your spouse, and dependents being claimed on tax return

  • IF YOU DO NOT HAVE A SOCIAL SECURITY NUMBER - Individual Taxpayer Identification Number (ITIN) assignment letter


Bring all items that apply to avoid delay of completing and E-filing your federal, state, and local tax return:

  • Birth dates for you, your spouse, and dependents being claimed

  • Wage, earning, and retirement statements (Form W-2, 1099-MISC, 1099-R)

  • Interest and dividend statements (Forms 1099-INT, 1099-DIV)

  • Unemployment Compensation and/or State Tax Refunds (1099-G)

  • Social Security statements (SSA-1099)

  • Gambling winnings Statements (W-2G)

  • Capital Gains/Loss (1099-B)

  • Cancellation of Debt statements (1099-C)

  • Education and Tuition statements (1098-E, 1098-T)

  • Sale of Home (1099-S)

  • Affordable Health Care statements (Forms 1095 A, B, or C)

  • Health Savings Acct (1099-SA)

  • Health insurance exemption certificate, if received

  • A copy of last year's federal and state tax returns, if available

  • Proof of bank account routing and account numbers if you want direct deposit/debit such as a blank check - No deposit slips

  • Total paid for daycare provider and the daycare provider's tax identifying number such as their Social Security number or Employer Identification Number

  • Total dollar amount of receipts if you are deducting expenses for business, medical, or other itemizing.  VITA volunteers are not responsible for calculating receipts.  (Be sure to retain your receipts for IRS auditing purposes)

  • Property Tax/Rent Rebate preparation- If you received SSI or SSD you must bring the award letter for the tax year being filed.
